Social determinants of health are non-medical factors — such as safe and affordable housing, access to healthy food and income inequality — that can significantly impact an individual’s overall health, well-being and medical outcomes. Designed for a wide range of clinicians, this course aims to increase knowledge and skill in recognizing social determinant of health issues and, where possible, helping patients navigate these challenges. This course will focus on tangible and doable practices that will support patients in improving their health across a variety of social determinant of health issues.

 


Objectives
At the conclusion of this activity, participants should be able to:

  1. Cite ways in which clinicians can help patients with lower socioeconomic means and improve their clinical care outcomes
  2. Identify the social determinants of health
  3. Consider patient concerns when asked to eat a healthy diet or improve medication adherence
  4. Apply questions and tools to identify and combat e-cigarette intake
  5. Discuss elements of the LGBTQ-Elder experience and review barriers to sucessful aging faced by LGBTQ Elders
  6. Review lead screening methodologies from a social determinants of health perspective, including access to safe water and housing
  7. Identify and respond to patients at-risk for trauma in childhood and adolescence
  8. Discuss provider and societal stigma towards patients with addiction as a social determinant of health
  9. Describe the key drivers for medication costs in the national and regional setting
  10. Summarize how to take a history for social determinants of health
