Overview

This course is about understanding what matters most to our patients facing serious illness and teaching the skills to take on these conversations with our patients, using Ariadne Laboratories Serious Illness Conversation Guide.

The day will start off with a discussion about the different advanced care planning approaches that exist, review the evidence behind them and pose a framework around what and when to use with your patients in order to best document their wishes.

Then we will move beyond documenting wishes to understanding how to elicit a patient’s personal values, preferences and priorities using the Serious Illness Conversation Guide Training.  This is the hallmark of providing whole person care: making sure that treatment decisions align with each patient’s values. The training is a 3 hour interactive curriculum designed to give you both an understanding of the benefits of using this guide and an opportunity to practice using it.

We hope this training will develop your skills and confidence in order to conduct more goals of care conversations with patients facing serious illness as well as documenting these conversations in the electronic medical record (EMR) using the Serious Illness Conversation Guide.

When you register for this course, you will choose to attend either the morning OR afternoon session of the Serious Illness Conversation Training. See schedule below. Each session is limited to 25 participants and will be conducted via livestream over Zoom.

Objectives
At the conclusion of this activity, participants should be able to:

  1. Describe the evidence-based benefits of serious illness conversations for patients and families.
  2. Describe the clinician’s role in improving serious illness conversations.
  3. List the components of the Serious Illness Conversation Guide.
  4. Practice using the Serious Illness Conversation Guide.

This course is open to VMFH clinicians. Training sessions are limited to 25 participants.

The Virginia Mason Medical Center is accredited by the Accreditation Council for Continuing Medical Education (ACCME) to provide continuing medical education for physicians.

The Virginia Mason Medical Center designates this live activity for a maximum of 3.75 AMA PRA Category 1 Credits™. Physicians should claim only the credit commensurate with the extent of their participation in the activity.

ABIM Maintenance of Certification (MOC): Successful completion of this CME activity, which includes participation in the evaluation component, enables the participant to earn up to 3.75 Medical Knowledge MOC points in the American Board of Internal Medicine’s (ABIM) Maintenance of Certification (MOC) program. Participants will earn MOC points equivalent to the amount of CME credits claimed for the activity. It is the CME activity provider’s responsibility to submit participant completion information to ACCME for the purpose of granting ABIM MOC credit.
